Directions

New users

  1. Create your login by entering a first name, last name and email.
  2. This information will be saved. Please use it anytime you want to access the form to update your program data. This also allows you to complete the survey in multiple sessions if needed or desired.
  3. You can always use this form to retrieve your data.

Returning users

  1. Use your personal login information (first and last name, and email address) to access your survey data. This information must match your original log in information exactly. If you need assistance, please request it using our feedback form.

Session expired message

  1. You may have been directed to this page because your session has expired, meaning you were logged on to the form, but had not requested a page or submitted data for over 1 hour.
  2. To resume your session, please log back on to the form using your personal URL or request it again following the directions above.

The purpose of this survey

The TIE has been tracking telemedicine activity since 1995. This survey has been the only systematic attempt to track telemedicine activity worldwide. The data you provide will give others doing research valuable insight into what constitutes the practice of telemedicine/telehealth. Additionally, the Telemedicine Research Center (TRC) is sponsoring an annual report that provides detailed statistical analysis of the data you provide. The report is filled with tables and figures suitable for manuscripts and presentations on telemedicine growth and activity. Please make sure your program or network is added to this valuable database of information.

About the form

Tips

  1. We estimate it will take you between 30 to 60 minutes to complete the survey.
  2. In order to expedite the process, we suggest you have any statistical data, such as number of consults, on hand before you start the survey. It is possible to submit the data you have available now and return later to complete skipped fields. However, some portions of the survey which are customized based on particular responses may be inaccessible until the needed data is provided.
  3. Required fields are signified by a red star *.
  4. Some required fields have special directions. They are indicated by two red stars **.
  5. If, based on your answers, a section of the survey does not apply to your program, you will automatically be directed to the next applicable section.

Technical

  1. Data is submitted on a page by page basis. To submit data click the submit and go button at the bottom of each page.

Sessions

  1. You can complete the survey in multiple sessions.
  2. A session starts from the time you log on to the form until either your session expires or you log off. In rare cases, your session could be interrupted due to server problems or maintenance issues. In this case any previously submitted data will still be saved, however any unsubmitted data on the current page may be lost.
  3. Any information you submit will be preserved for later sessions. However, please be aware that if you enter data into the form fields and allow your session to expire before you click the submit button (or if you never click the submit button), the unsubmitted data will be lost.

Survey data

As part of an agreement with the National Library of Medicine (NLM), the TIE makes all of its telemedicine program data available to the NLM's Health Services Research Projects in Progress (HSRProj) database).

The information you provide may also be used in future productsoffered for sale, such as the 2004 TRC Report on US Telemedicine Activity. In such case, you would be offered a substantial discount on the product(s).
