Vice President, Strategic Business Development and Consumer Innovation, UPMC Health Plan

    Kim Jacobs is the Vice President, Strategic Business Development and Consumer Innovation. Jacobs joined the UPMC Health Plan in May 2006 and is responsible for optimizing member/consumer engagement using relevant digital technologies to maximize consumer health and productivity engagement and outcomes within a Population Heath Management (PHM) framework.

    Jacobs also oversees the strategic development of HealthPlaNET, a proprietary care management suite currently used to manage approximately 8 billion in UPMC Health Plan risk premium. HealthPlaNET is an innovative, person-centric information hub, integrates a myriad of data feeds to promote a strategic 360 approach to care management; supports a robust analytics engine, with intelligent prioritization; stratification for prioritization of care; highly configurable rules, and logic to deliver action insights and information to over 1,600 users. Additional, areas of focus include strategic business assessments of potential partnerships and/or commercialization of UPMC Health Plan assets. Prior to joining UPMC Health Plan, Jacobs served as the President of Chartwell Home Therapies, L.P. a company with $300M annual revenues with fourteen (14) academic medical centers as major clients. Over the past 43 years Jacobs has served as a Chief Operating Officer, Executive Vice President Sales, Vice President of Business Development and Strategic Alliances, Vice President Operations, and an Executive Director in both the private and public healthcare services sector. Jacobs has a BA Psychology from The Citadel, Charleston S.C. and Executive MPA studies program, Georgia State University, Atlanta, Georgia. Jacobs is a graduate of both UPMC’s Leadership Development Initiative (LDI) – 2009, and Leadership Pittsburgh, Inc., Leadership Pittsburgh program, Class XXVIII-2011, and serves on the 2016-2017 Leadership Pittsburgh, Inc. board. Jacobs was also appointed in 2016 to the Pennsylvania Insurance Departments Price and Quality Transparency initiative exploring ‘shoppable care’ options for Commonwealth of Pennsylvania citizens.
